Uscgrim is a psychiatric patient in Pillars of Eternity. He can be found in the North Ward of the Sanitarium in the Brackenbury district of Defiance Bay (in the cell at the end of the hall).

A willowy adolescent boy of seventeen stands in the center of his cell, gawkish and stoop-shouldered. He stares straight ahead, the occasional blink his only movement, and it is unclear whether he registers your presence at all behind his glassy, doll-like eyes.
Uscgrim was admitted for episodes of catatonia followed by occasional violent outbursts, a result of severe soul fragmentation and essence deficiency.
It made him the perfect vessel for Thaos, as part of his plan to humiliate animancy.

- The Man Who Waits: The focal point of the quest, Uscgrim acts as a vessel for Thaos and a lure for the Watcher. His parasitic soul allows to hold the Watcher and prevent him from escaping once he connects with Uscgrim's soul.
